Noticeboard

Our Vaccination Teams are working tirelessly to ensure patients are vaccinated as swiftly as possible. We work with 7 other practices as Swift Primary Care Network (PCN) offering vaccinations from our Vaccination Site at Knowle West Healthy Living Centre. We also work with our neighbouring PCN, Bridgeview Medical, to ensure our patients have the best possible opportunities for vaccination.  We are making great progress. Most of our patients over 75 have had a first vaccination and we are now moving into the next groups (over 70's & clinically extremely vulnerable).

We will contact you as soon as we can offer you a vaccination and, on occasion, you may hear directly from Bridgeview Medical.

We are not linked with the NHS letters you may receive offering vaccinations at Ashton Gate but of course you should make the arrangements that best work for you.

New Patient Registrationregistration.jpg

If you expect to reside in this area for more than 3 months you are advised to register with a practice. If you have been told by the practice that you are within our catchment area (see map below) and you wish to register here, then call into the surgery where you will be asked to complete a registration form. Alternatively you may print off a registration form, fill it out and bring it in to the practice.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take considerable time for us to receive your medical records. You may print off a medical questionnaire and bring it to the practice together with your registration form.

When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

When you come in, please remember to bring:

  • details of your last address and your last doctor's name and address, which will be needed to complete the registration form
  • your NHS number if you have it (this makes getting you registered very much faster)
  • your exact date of arrival if you are new in this country
  • finally, and most importantly, we ask to see photographic proof of identity (or Birth Certificate) and evidence to confirm your address within the last 3 months

Once the registration form and medical questionnaire have been received and your ID checked, it will take 48 working hours for the request to be processed.

When registration is complete, your medical records will be sent for and these will stay with our practice until you move on again.

Medical treatment is available from the date of registration.  Please contact reception for further information.
